Closing the Gap Care Coordinator (Country)

We are seeking enthusiastic Care Coordinators to work in our Mid-North/Yorke Peninsula, Country Closing the Gap team.

The Care Coordinator will contribute to improved health outcomes for local Aboriginal and Torres Strait Islander people living with chronic health conditions. The Care Coordinator will be responsible for working collaboratively with patients, general practitioners, practice staff and Aboriginal health services to coordinate the appropriate care and services.

The successful applicant will have current AHPRA registration and experience working with Aboriginal people; case management, care coordination or care planning experience; and an understanding of and proven ability to work with GPs, primary health nurses and other allied health providers.

headspace Youth Worker

Do you want to make a difference to the lives of young people?

We are seeking a passionate and skilled professional to join our dynamic headspace team, assisting in the engagement, brief assessment, care co-ordination and youth work intervention of young people aged 12-25 with mental health, general health, vocational and/or AOD concerns within an innovative early intervention framework. You will develop, implement, and review a range of community engagement and youth participation strategies.

You will possess demonstrated skills in youth engagement, brief psycho-social assessment, care-coordination, and the provision of short-to-medium term youth work interventions.
