Closing the Gap Care Coordinator

We are seeking enthusiastic Care Coordinators to work in our Northern Adelaide Closing the Gap team.

The Care Coordinator will contribute to improved health outcomes for local Aboriginal and Torres Strait Islander people living with chronic health conditions. The Care Coordinator will be responsible for working collaboratively with patients, general practitioners, practice staff and Aboriginal health services to coordinate the appropriate care and services.

The successful applicant will have current AHPRA registration and experience working with Aboriginal people; case management, care coordination or care planning experience; and an understanding of and proven ability to work with GPs, primary health nurses and other allied health providers.

Closing the Gap Outreach Worker

We are seeking enthusiastic an Aboriginal Outreach Worker that will join the Closing the Gap team that provides support in Northern Adelaide.

The Aboriginal Outreach Worker will contribute to improved health outcomes for local Aboriginal and Torres Strait Islander people living with chronic health conditions. The Aboriginal Outreach Worker will work alongside CTG Care Coordinators and will be responsible for working collaboratively with patients, GPs, practice staff and Aboriginal health services to coordinate patient care and wellbeing.

The successful applicant will have experience working with Aboriginal people and have demonstrated expertise in community liaison activities such as establishing links between local Aboriginal and Torres Strait Islander communities, health services and mainstream primary care services.

The successful applicant must also have experience identifying access barriers – working with the Indigenous CTG team to provide feedback regarding access barriers to primary care services for Aboriginal and Torres Strait Islander people, and work to overcome these barriers.

HEAL Project Officer

We are seeking an enthusiastic HEAL Project Officer to join the Community Health Stream that administers the HEAL Program across various North-Western Country regions.

The HEAL™ Project Officer is responsible for administering the Healthy Eating Activity and Lifestyle (HEAL™) Program across the Gawler/Barossa, Yorke Peninsula, Mid-North, Flinders & Outback, Eyre Peninsula and Far Western regions. The HEAL™ program is a lifestyle modification program facilitated that supports participants to develop lifelong healthy eating and physical activity behaviours.

The successful applicants will have experience working within a multi-disciplinary team, with needs assessment and planning, developing multi-programmes approaches and cross-sector linkages, and implementing strategies to improve engagement with primary care providers.

Education Officer

We are seeking an enthusiastic Education Officer to join the Community Health Stream that facilitates accredited Education workshops across Northern Adelaide and Central-Western Adelaide regions.

The Education Officer is responsible for the delivery of high quality education and support to our practices, community and stakeholders. This is done within the Education Service team to plan, secure and coordinate relevant education and events each year based on needs assessment in accordance with our annual plan, accreditation and funding requirements.

The successful applicants will have experience working with needs assessment and planning, compliance with accreditation and organisational policies, and identifying topics that aim to build the capacity of GPs and primary care providers.
